First you make an Initial Consultation appointment. We ask that you fill out the necessary intake paperwork (about 5 pages).The intake forms can be faxed to you, picked up in person, or downloaded from this web site. This paperwork lets us learn a lot about you at a glance so that during the time you are in our office we can get more specific and ask the questions that pertain just to your condition. On this first visit we do an Initial interview , body assessment, and allergy screening. You are then scheduled to return in a few days for a Report of Findings . At the Report of Findings we present a detailed program designed just for you.
